50 / 50 Withhold
Half is paid monthly. The other half is released when your organization hits the Outcome Attainment Threshold — 50% of patients meeting clinical targets in Year 1.
Co-Management Pay
PCPs and referring clinicians earn ~$30 per documented care coordination review, plus a ~$10 onboarding add-on — up to ~$100 per beneficiary per year. No patient cost-sharing.
Coinsurance Waiver
Organizations may uniformly waive the 20% beneficiary co-insurance on outcome-aligned payments as an engagement incentive under a CMS safe harbor.

CMS Adds Heart Failure, COPD, Substance Use Disorder & Tobacco Cessation Tracks
Starting April 1, 2027, ACCESS adds four new clinical tracks plus a follow-on period for chronic musculoskeletal pain. Current participants and applicants do not need to reapply. CMS has not yet published outcome measures or payment rates for the new tracks. ACCESS launched July 5, 2026 with more than 160 participating organizations.
